What to Expect (Job Responsibilities):
- Provide outreach to members via phone or home visits to discuss care/service plans and ongoing education
- Coordinate care activities with healthcare providers and community partners to ensure continuity of care
- Address member and provider inquiries related to care/service plans
- Document and maintain member records in compliance with regulatory requirements
- Identify member needs and make appropriate referrals to care managers and community organizations
What is Required (Qualifications):
- High School diploma or GED
- 1-2 years of related experience in care coordination or healthcare service
- For Arkansas Total Care plan: Bachelor's degree in social science/health-related field or high school diploma with at least one year of experience in care coordination for specific populations
- Valid driver's license and successful completion of child and adult maltreatment checks and drug screening
- Must reside in Arkansas or a bordering city
How to Stand Out (Preferred Qualifications):
- Experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients
