What to Expect (Job Responsibilities):
- Provide case management services to patients, families, and caregivers
- Develop and monitor multidisciplinary collaboration of services for patients
- Assist adult patients and their families with personal and environmental difficulties
- Initiate care plans and conduct both subjective and objective assessments
- Utilize community health resources to support patient needs
What is Required (Qualifications):
- Bachelor’s Degree in Health & Human Services or a related field
- Minimum of 1 year of experience in Long Term Care and Health Plans
- Strong understanding of case management and care coordination
- Ability to conduct assessments and initiate care plans
- Experience with community health resources is beneficial
How to Stand Out (Preferred Qualifications):
- Experience in nursing home settings
- Background in case management or care coordination
- Familiarity with community health resources
- Experience initiating care plans
- Additional certifications in relevant fields
