      What to Expect (Job Responsibilities):
      - Provide case management services to patients, families, and designated caregivers
      - Develop and monitor multidisciplinary collaboration of services for patients
      - Assist adult patients and families with personal and environmental difficulties related to medical conditions
      - Conduct assessments and initiate care plans based on patient needs
      - Engage with community health resources to support patient care

      What is Required (Qualifications):
      - Bachelor’s degree in Health & Human Services or a related field
      - Minimum of 1 year of Long Term Care and Health Plans experience
      - Experience with nursing home settings is preferred
      - Background in case management or care coordination is preferred
      - Familiarity with community health resources and assessment techniques is preferred

      How to Stand Out (Preferred Qualifications):
      - Experience initiating care plans and conducting both subjective and objective assessments
      - Previous nursing home experience
      - Knowledge of community health resources
      - Background in case management or care coordination
      - Additional certifications related to healthcare services
