      What to Expect (Job Responsibilities):
      - Assess and evaluate member needs to facilitate optimal wellness and access to care
      - Develop trusting relationships with members to improve care quality and coordination
      - Coordinate necessary medical and non-medical services for members
      - Collaborate with interdisciplinary care teams to address members' needs
      - Serve as the primary point of contact for members and coordinate care with various providers

      What is Required (Qualifications):
      - Licensed Registered Nurse (RN) in the state of Indiana without restrictions
      - Minimum of 2 years of clinical experience in case management or related healthcare services
      - Proficient in Microsoft Word, Excel, and Outlook
      - Strong communication and interpersonal skills to build rapport with customers and stakeholders
      - Ability to manage multiple priorities in a fast-paced environment

      How to Stand Out (Preferred Qualifications):
      - Bilingual (English/Spanish) or (English/Burmese)
      - Experience in nursing home diversion, long-term care, or case management
      - Familiarity with Medicare and Medicaid recipients
      - Experience working with a geriatric population
      - Knowledge of community health resources and social service agencies
