What to Expect (Job Responsibilities):
- Perform case management services for patients, families, and designated caregivers
- Develop and monitor multidisciplinary collaboration of services for patients
- Conduct in-person face-to-face assessments, traveling 3-4 days per week
- Assist adult patients and their families with challenges related to medical conditions
- Initiate care plans and conduct both subjective and objective assessments
What is Required (Qualifications):
- Bachelor's Degree in Health & Human Services or a related field
- Minimum of 1 year of experience in Long Term Care and Health Plans
- Experience with community health resources
- Familiarity with case management and care coordination
- Ability to conduct assessments and develop care plans
How to Stand Out (Preferred Qualifications):
- Experience in nursing home settings
- Previous case management or care coordination experience
- Knowledge of community health resources and services
- Background in initiating care plans and conducting assessments
