      What to Expect (Job Responsibilities):
      - Conduct face-to-face assessments with patients, families, and caregivers
      - Provide case management services to assist patients in navigating health-related challenges
      - Develop and monitor multidisciplinary collaboration of services for patients
      - Initiate care plans and perform both subjective and objective assessments
      - Assist adult patients and their families dealing with personal and environmental difficulties related to medical conditions

      What is Required (Qualifications):
      - Bachelor’s degree in Health & Human Services or a related field
      - Minimum of 1 year of experience in Long Term Care and Health Plans
      - Ability to travel for in-person assessments 3-4 days per week
      - Knowledge of community health resources
      - Experience in case management or care coordination

      How to Stand Out (Preferred Qualifications):
      - Nursing home experience
      - Experience initiating care plans
      - Familiarity with community health resources and support services
      - Previous case management or care coordination experience
      - Experience with assessments in healthcare settings
