What to Expect (Job Responsibilities):
- Utilize care management assessments to evaluate members' healthcare needs and recommend appropriate interventions
- Identify high-risk factors and service needs to implement proactive support for members
- Collaborate with multidisciplinary teams to enhance medical appropriateness and quality of care
- Engage and empower members in their healthcare decisions through coaching and motivational interviewing
- Ensure compliance with regulatory and accreditation guidelines while documenting care processes
What is Required (Qualifications):
- Minimum 6 months of experience in foster care, juvenile justice systems, or behavioral health services
- At least 2 years of experience in social services, psychology, or counseling
- Minimum 1 year of experience using MS Office Suite applications, including Word and Excel
- Must be willing to travel 80-90% of the time to meet members in person
- Candidate must reside in one of the specified counties within the assigned region
How to Stand Out (Preferred Qualifications):
- 3+ years of experience in behavioral health, social work, or counseling
- Experience with Managed Care Organizations (MCOs)
- A Master's degree in a relevant field
