      Overview

      Come join a dynamic care team at LifeLong Medical Care. We are looking for a Case Manager (CM) for the Care Neighborhood (CN) program. Care Neighborhood is an innovative and interdisciplinary program aimed at supporting and empowering identified high risk-high needs patients in improving their health outcomes. This Case Manager (CM) position will be based out of the TRUST clinic, a primary care clinic in Downtown Oakland providing medical, behavioral health, and social service support to patients experiencing homelessness and housing insecurity. The multidisciplinary team (RN, MD, MSWs, LCSWs, and CMs) works together to support patients and address the social determinants that impact health. Activities include outreach and engagement, development of the care plan, linkage of the patient with resources (food, housing, transport, financial) as well as linking the patient with care within the clinic.

      Ideally, the CN CM will be a trusted member of the community in which they will work. This relationship will enable the CM to serve as a liaison between health and social services and the clinic. Much of this work will take place in various settings – medical office, specialists, hospitals and patients’ homes (including shelters, encampments, and transitional housing programs).

      This position functions in partnership with Community Health Center Network (CHCN) in San Leandro. There will be a period of orientation with the central team at CHCN as well as internal orientation support from LifeLong. All of this will happen virtually however in-person biweekly trainings in San Leandro may commence pending improved public health and safety conditions.

      This is a full time, 40 hrs/wk, benefit eligible position.

      This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

      Benefits

      Responsibilities

      • Advocates for patients and helps them achieve their best health.
      • Functions as a trusted member of the community.
      • Conducts outreach and enrollment assessments.
      • Meets with patients in various settings – medical office, specialists, hospitals and patients’ homes including weekly check-ins in San Leandro.
      • Creates a care plan that reflects patients’ needs and connects patients to resources to meet these needs.
      • Communicates with patients, documents clearly in the medical record and interfaces with medical providers to advocate for the patient.
      • Serves as a liaison between the community and health and social services.
      • Interfaces well with clinic staff, community and Care Neighborhood team.

      Qualifications

      • Experience working with underserved communities, including individuals experiencing homelessness and/or housing insecurity. Commitment to working directly with low-income persons from diverse backgrounds in a helpful and supportive manner.
      • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change while maintaining a positive attitude.
      • Ability to prioritize tasks, work under pressure and complete assignment in a timely manner.
      • Ability to effectively present information to others, including other employees, community partners and vendors.
      • Ability to seek direction/approval from on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
      • Work in a team-oriented environment with a number of professionals with different work styles and support needs.
      • Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally sensitive.
      • Conduct oneself in external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and sensitive staff.
      • Ability to see how one’s work intersects with that of other departments of LifeLong Medical Care and that of other partner organizations.
      • Make appropriate use of knowledge/ expertise/ connections of other staff.

      Job Requirements

      • High School diploma or GED.
      • At least three (3) years of progressively responsible work experience in a community-based health care or social service capacity.
      • Knowledge of East Bay health and social service resources.
      • Previous work providing services to persons who are experiencing homelessness, living with a disability, using substances, and experiencing mental health challenges.
      • Proficient in Microsoft office word with ability to manage databases.
      • Reliable means of transportation for community outreach and travel to patient appointments and meetings. Note: staff will never be required to transport patients in their personal vehicle

      Job Preferences

      • Bachelor’s Degree in Social Work, Health or Human Services field.
      • Bilingual in English/Spanish strongly preferred.
      • Access to use of personal vehicle and in possession of current license and insurance.
