- Current valid license as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in accordance with State of Residency.
- Preferred 1-3 years' experience as an LVN or LPN in public health, acute care, case management and/or in-home health care.
- Strong communication skills, both oral and written.
- Critical thinking skills to appropriately resolve inquiries, escalated cases and orchestrate holistic care.
- Experience in conflict resolution and providing one-call resolution.
- Must pass an assessment test.
- Bilingual English/Spanish is a plus.
- Self-starter and a team player with strong organizational skills.
- Must have a high degree of patience and interpersonal skills.
- Excellent customer service skills.
- Ability to work independently or in a team.

Job Summary
The Nurse Case Manager is responsible for providing confidential, unbiased assistance to eligible LIBERTY Dental Plan (LIBERTY) enrollees of State Medicaid programs in need of dental services who also have a co-morbidity of special health care needs. Responsibilities also include, and are not limited to, assisting the Coordinators Care Management (CCMs) to perform their professional functions.
What we are looking for:
An experienced Licensed Practical Nurse (LPN) located within the Tampa Bay market with the ability to work collaboratively and interface with multiple entities in support of providing member care services to include plan benefits, care coordination, and enroll dental services education. A Licensed Practical Nurse (LPN) in accordance with State of Residency is required. The ideal candidate will have experience in public health, acute care, case management and/or in-home health care.

In the spirit of pay transparency, we are excited to share the base salary for this position is $28.25 – $36.72 per hour, exclusive of fringe benefits or potential bonuses. If you are hired at LIBERTY, your final base salary compensation will be determined based on factors such as geographic location, skills, education, and/or experience. In addition to those factors – we believe in the importance of pay equity and consider internal equity of our current team members as a part of any, final offer. Please keep in mind that the range mentioned above is the full base salary range for the role. Hiring at the maximum of the range would not be typical in order to allow for future & continued salary growth. We also offer a generous compensation and benefits package (more information on benefits listed below).
