What to Expect (Job Responsibilities):
- Empower caregivers and families to support optimal wellness and access necessary services and community resources
- Assess, plan, and implement individualized care strategies for patients
- Build and maintain relationships with caregivers and healthcare providers to develop member-focused plans of care
- Advocate for patients and families to ensure their needs and choices are fully supported by the healthcare team
- Identify and address any barriers to member progress
What is Required (Qualifications):
- Minimum of 2 years of experience working in managed care or a relevant clinical environment, preferably with a neonatal population
- Valid unrestricted LCSW Social Worker license in SC, FL, or TX preferred
- Willingness to obtain licensure in other states as required by client contracts
- Experience working with a Medicaid population preferred
- Proficiency in PC-based word processing and database documentation, as well as managed care systems or EMRs
- Excellent communication skills and organizational ability
- Self-motivated and willing to learn multiple tasks
- Well-organized and able to prioritize tasks
- Accurate spelling and documentation
How to Stand Out (Preferred Qualifications):
- Fluency in Spanish
- CCM certification preferred upon hire
- Background in effecting change to overcome social determinants of health
