Job Description

- Act as a liaison in partnerships with high level community service providers, bringing a comprehensive and individualized healthcare lens on cross agency coordination and projects such as with Community Behavioral Health (CBH) and the Integrated Care Plans (ICP)
- Effectively lead, execute, and support multi-agency projects, research, and management analyzing participant feedback to enable teams to achieve project outcomes and to ensure the company accomplishes deadlines, such as Tenant Service Coordination (TSC) bimonthly meetings, Journey of Hope (JOH), etc.
- Support PMHCC Case Management with administrative functions that include carrying a case load of 15-25 cases that derive from referrals, completing activations and discharges, and assessing cases for warm hand-offs
- Able to work in a Fee-for-service environment producing a minimum of 75% monthly available billable units which are non-inclusive of back-out hours.
- Develop tools and strategies to connect participants with resources based on the six dimensions of ASAM (American Society of Addiction Medicine)
- Develop a universal, cross-referenced resource catalog and regularly update to reflet current status
- Develop unique ideas and projects to ameliorate and address social determinants of health that make the work more efficient, useful, accurate, and productive
- Assist PMHCC Case Management with refining existing tools, protocols, and programming while creating new programming as needed
- Participates in designated agency and external meetings; attends in-service and other training in order to meet mandated training hours
- Provide support as needed to PMHCC CM Program Director and PMHCC Case Management as deemed by PMHCC CM Program Director and Senior Management
- Excellent verbal and written communication skills: Must be able to communicate effectively with all staff members across all levels of the organization and advocate on behalf of our participants to stakeholders in a respectful and thoughtful manner

- Ability to work effectively, both independently and collaboratively, with cross systems/teams and to develop productive relationships with a diverse group of colleagues, consultants, academic partners, and others
- Ability to think critically and clearly about case manager challenges, participant challenges, and provider challenges; and exercise sound insight and judgment when problem solving to develop innovative solutions
- Ability to develop and present concepts to a range of key stakeholders, including executive leadership
- Ability to prioritize and manage multiple ongoing projects while meeting deadlines
- Ability to utilize technology in innovative and creative ways to improve access to behavioral health resources
- Must display a mastery of the following case management core competencies:
- Communication skills, team building and leadership, compassion, time-management and organizational skills, problem-solving skills
- Ability to be to work with flexibility, efficiency, and diplomacy, both individually and as part of a team, in handling changing priorities in a fast-paced work environment
- Preferred Master's level degree in a behavioral health field with three years of work experience in social services or Bachelor’s Degree in a behavioral health field with three years of work experience in social services. An Associate’s Degree with a minimum of five years’ work experience.
- Good clinical and assessment skills
- Knowledge and experience addressing the social drivers of health.
- Expertise in two or more of the following required: mental health, criminal justice system, substance use, homelessness
Position Summary:
PMHCC Case Management is a unit dedicated to providing recovery support services to individuals as they journey through their recovery process from substance misuse and/or cooccurring disorders, with the goal of achieving and maintaining long-term recovery. As case managers work with individuals in their recovery process, individuals are empowered to identify their strengths and talents and focus on the social determinants of health, or areas of need that he/she/they wants to address at specific points in time. This collaborative approach is built upon the idea that all individuals, regardless of their past, their challenges, and their walks of life, are people first. The PMHCC-CM unit is seeking to expand capacity under the guidance of PMHCC, Inc., Department of Behavioral Health and Intellectual DisAbility Services (DBHIDS) and Community Behavioral Health (CBH) to provide and revolutionize services to all Philadelphia County Residents who are uninsured, underinsured, and those that are health choices eligible.
The Case Manager Tech will be responsible for providing programmatic support to the Case Management team by using harm reductionist frameworks, and managing initiatives within the unit that cut across multiple City agencies and affect the lives of the city’s most vulnerable residents. This position will coordinate internal and external partnerships that promote harm reduction strategies and facilitate access to resources, services, and treatment for people who use drugs. This position will help lead the PMHCC Case Management team to increased production of quality engagement billable services, enhanced resolution of problems and barriers to services, opportunity for linkages, and promotion of advocacy efforts, by analyzing data, creating reports, and developing strategies to improve program performance and outcomes. The Case Manager Tech will establish and coordinate projects aiming to address social determinants of health and to provide support to individuals who have extensively utilized treatment and social service agencies.
