      Overview

      Under the supervision of the PACE Regional Director of Senior Care Services, responsible for the overall operations of the SBC PACE center. Works with key management personnel, in achieving services according to licensing standards and/or regulatory agency standards, which are applicable to PACE.

      Responsibilities

    • Primary accountability for the planning, coordination and implementation of all activities in the facility.
    • Coordinates supervision of the SBC department managers including nursing, social workers, nutrition, activities, rehabilitation, contracted personnel and administrative support staff while directing the daily operations of the SBC center.
    • Orient new supervisors to PACE Program components and their specific job functions.
    • Coordinate with PACE Regional Director of Senior Care Services to prepare and review required reports and billing procedures within specified time frames for AltaMed Corporate and government agencies.
    • Provide facilitation and leadership to the Interdisciplinary Team (IDT) meetings including the assignment of newly identified action items and verification of action item completion on a weekly basis.
    • Review monthly operation expenses to assure budgetary compliance and to identify any budget variations as appropriate.
    • Attend any required SBC Committee meetings including QUMC, and Safety Committees.
    • Chair the SBC Center Safety Committee, General Staff meetings and Participant Advisory Committee.
    • Responsible for ensuring the maintenance of day center physical plant and equipment.
    • Participate in budget planning process and review.
    • Responsible for the coordination of overall operations of the SBC Clinic, including but not limited to: Hospitalizations; Internal/External clinic encounters as well as appointment tracking; maintenance of mortality data, infection control, and communicable disease tracking reports.
    • Monitor the activities of the IDT to ensure appropriate coordination and utilization of health care services: Daily resolution and follow through; Daily IDT Issue Log; new assessments; re-assessments; care plans; progress notes; and family conferences.
    • Responsible to ensure compliance in all areas of program operations and to prepare the necessary Corrective Action Plan (CAP) as needed in a timely process. CMS; DHCS, Clinic; DHCS, Office of Long Term Care; AltaMed and other regulatory agencies as needed)
    • Under the supervision of the PACE Regional Director of Senior Care Services, ensure compliance in all areas of personnel management including recruitment, timely employee evaluations, competency assessments, annual physicals and TB clearance, daily coverage, training and staff development, CEU’s, and disciplinary actions in accordance with our corporate policies and procedures.
    • Responsible to investigate, monitor, and track all SBC participant grievances and incidents to ensure prompt resolution to their issues.
    • Responsible to provide oversight to all corporate-wide and SBC policies and procedures and ensure all files updated.
    • Qualifications

    • Bachelor’s degree in Social Work, Nursing, Psychology or Gerontology required. Master’s degree preferred.
    • Experience in working with Senior Care Services and elderly population.
    • Experience working collaboratively with the following disciplines: nursing, social work, psychology, recreation, occupational therapy, physical therapy, speech therapy, dietetics, and gerontology.
    • Minimum of three years’ experience in supervisor or management position in healthcare or related field.
    • Bilingual: English/Spanish/Mandarin/Cantonese depending on location preferred.
