- Chart Research Escalation Specialist is responsible for reviewing medical records for the necessary documents for proper indexing
- Serves as Subject Matter Expert (SME) for Chart Research Department. Assists with answering questions from team
- Handles escalated issues from team
- Documents work processes as required. Supports the training team with continuing education for new and tenured associates
- Assists with quality assessments
- Provides coaching to team members as needed
- Ensure medical records are complete within the electronic medical records system by identifying and obtaining any missing chart information including signatures from physicians and other pertinent personnel
- Conducts Review of Systems (ROS), Exam, HPI, and MD Notes
- Retrieves existing medical records from Electronic Medical Records (EMR) system
- Scans and inputs documents
- Verifies that information is indexed appropriately
- Pulls down medical records through Automate Tasks
- Monitors the performance of the Automate Tasks
- Logs into hospital systems and downloads registration logs and demographic data
- Create new entries and sub files as needed with correct medical record number
- Performs special projects and other duties as assigned
- High School Diploma or GED
- At least six (6) months of experience in handing patient health information and/or medical records
- Working knowledge of medical terminology, anatomy and physiology, legal aspects of health information
- Working knowledge of department roles and responsibilities
- Knowledge of Electronic Medical Records (EMRs) such as Cerner, Meditech, Epic, PICIS
- Strong critical thinking skills
- Strong organizational skills
- Strong time management skills
- Strong word processing, spreadsheet, database, and presentation software skills
- Ability to read, understand, and apply state/federal laws, regulations, and policies.
- Ability to communicate with diverse personalities in a tactful, mature, and professional manner.
- Ability to remain flexible and work within a collaborative and fast paced environment.
- Basic use of computer, telephone, internet, copier, fax, and scanner.
- Basic touch 10 key skills.
- Basic Math skills.
- Understand and comply with company policies and procedures.
- Strong oral, written, and interpersonal communication skills.
- Strong time management and organizational skills.
- Base Compensation will be based on various factors unique to each candidate including geographic location, skill set, experience, qualifications, and other job-related reasons.
- This position is also eligible for a discretionary incentive bonus in accordance with company policies.
