      Job summary

      There is an exciting opportunity for a qualified nurse with experience of working with children and young people with mental health and/or a learning disability available to join the children's continuing care team. For the role of a children and young people's health assessor for 37.5 hours per week on a permanent basis with the team based in Newburn.

      The children's continuing care team consists of over 15 staff and are part of the All Age Continuing Care team for the North East including Tees, Durham, Sunderland, Tyneside, Gateshead and Northumberland. The CCC service undertake assessments across the North East as outlined in the National Frameworks for Children's Continuing Care (CCC) utilising a partnership approach. The children and young people's health assessor role have a key role in leading the multi-disciplinary assessments, case managing and commissioning packages of care for children and young people eligible for CCC funding. The health assessor work in partnership with children, young people and their representatives, other professionals including health, education, and social care. The health assessor will also be supporting the expanding team and contributing to the achievement of the key performance indicators (KPI) for the ICS by acting as the first point of contact for children and young people whilst maintaining patient confidentially at all times.

      Main duties of the job

      This role will be responsible for:

      Clinical assessments of the health needs of children and young people to determine eligibility for CCC funding.

      Coordinating and completing MDT assessments and reviews in accordance with the National Framework.

      Provide a high standard of holistic health assessments within the agreed timeframes.

      Commissioning of the packages of care to meet the unmet health needs of children and young people eligible for CCC.

      Supporting children and young people with the offer of a personal health budget to meet the health outcomes.

      Case management for children and young people eligible for funding via CCC.

      Record keeping as per NECS policy and procedures.

      Working in partnership with children, young people, and their representatives.

      Building and maintain relationships with Local Authorities, providers, Foundation Trusts, and the voluntary sector.

      Contributing to the CCG's and ICB's achievement of their statutory duties.

      Supporting the transition of young people to adulthood.

      Contributing to the CCG/ICS statutory duties for SEND.

      Ensure that the health care needs of children and families eligible for Continuing Care are identified and met in accordance with the National Framework.

      Support the Lead Specialist Nurse and Continuing Care Strategic Lead in the development of a range of services for children with complex health needs including palliative care services.

      Undertake specialist nursing assessments for children with complex needs in relation to care packages, respite care and placements for continuing care.

      Collate and undertake comprehensive and coordinated assessments for decision making for NHS Continuing Care with children and families within health and social care and care plan and commission services to meet the needs of eligible clients.

      To be instrumental in the development and maintenance of effective systems, procedures and pathways to support Continuing Care and Personal Health Budgets.

      To promote Personal Health Budgets (PHBs) ensuring integration into continuing care and that they are offered where requested to those clients who meet agreed criteria.

      Attend the panels where arranged to present own cases and provide clinical information.

      Case manage continuing care clients ensuring timely reviews are conducted to ensure health and nursing care needs are being met appropriately and to ascertain care packages continue to be effectively commissioned and are cost effective.

      Assist in maintaining a database for all patients assessed for continuing care and ensure high quality assurance systems are in place.

      Develop strong working relationships with partner agencies including Acute Hospital Trusts, local authorities, the Independent Sector and others.

      Provide professional nursing advice to Acute Hospital Trusts, nursing homes and social services with regards to Continuing Care and Personal Health Budgets.

      Work with the continuing care team, ensuring that the reviews are carried out. Following the national framework with regards to time lines of assessment and reviews. There maybe a need to refer to the continuing care manager.
