      Job summary

      We are looking for a focussed, driven and forward-thinking system changer who has a real passion for making a difference to work with our children, young people and their families and carers in a person-centred way, to help bring their voices to life and achieve their outcomes. This exciting role is suitable for a wide range of practitioners, qualified or unqualified (with suitable demonstrable experience).

      Successful candidates will:

      Have relevant experience of working with autistic children and young people and/or a learning disability

      Be passionate about working co-productively and have experience of contributing to personalised care plans

      Have a good understanding of working in partnership and developing positive relationships

      Be assertive to appropriately challenge the systems and processes when they are not working

      Be innovative with the ability to think out of the box using creative solutions to prevent further escalation, family or placement breakdown and admission into an inpatient setting

      Base/Location: One of the 4 NHS South Yorkshire Bases (Westgate Barnsley, Sovereign House: Doncaster, Riverside House, Rotherham or Eyre Street, Sheffield), with regular travel across South Yorkshire and including hybrid arrangements working from home.

      Main duties of the job

      The focus of the job role is to ensure the system is cohesively working together to ensure children, young people, carers and families receive the right care and support, from the right person at the right time including:

      • Providing emotional and behavioral support around complex needs, ensuring clear communication with all parties involved.
      • Advocating in care discussions, maintaining accurate records of interventions.
      • Building positive relationships with various professionals to prioritise home and community-based care.
      • Troubleshooting and challenging practices to ensure appropriate and necessary support within the community.
      • Providing evidence-based challenges to practices and decisions, identifying areas for improvement.
      • Facilitating transitions into and out of inpatient settings, coordinating care across health, education, and social care.
      • Communicating sensitive and complex information regarding their care in an empathic and supportive way.

      NHS South Yorkshire Integrated Care Board oversees health and social care for a population of 1.4m people.
