What to Expect (Job Responsibilities):
- Conduct telephonic medication consultations meeting/exceeding production & quality metrics
- Complete annual medication reviews with emphasis on patient assessment and counseling
- Assess patients for medication adherence barriers and develop a plan for success
- Deliver evidence-based therapeutic recommendations to providers
- Maintain a high level of clinical knowledge of chronic disease states
What is Required (Qualifications):
- Doctor of Pharmacy degree or equivalent experience
- Active, unrestricted pharmacy license in the state of primary residence
- 3+ years of Clinical Pharmacy Practice experience
- 1+ years of Call Center experience
- Proficiency in Microsoft Word, Excel, Outlook, PowerPoint
How to Stand Out (Preferred Qualifications):
- MBA/MPH Degree
- Post Graduate Training (Pharmacy Residency and/or Fellowship)
- Bilingual with preference given to Spanish (Vietnamese, Cantonese, Mandarin, & Russian)
- Board certification by the Board of Pharmacy Specialties (BPS)
- Certified Geriatric Pharmacist
