What to Expect (Job Responsibilities):
- Perform medication review to identify medication-related problems and optimization opportunities
- Collect relevant information to understand the patient's history and clinical status
- Assess medications for appropriateness, effectiveness, safety, and adherence
- Develop pharmacist care plans based on evidence-based recommendations
- Implement recommendations in collaboration with the patient's care team
What is Required (Qualifications):
- Doctor of Pharmacy (PharmD) degree OR 10+ years of clinical experience
- Active, unrestricted Pharmacist License in applicable state(s) and willingness to get licensed in additional states as requested
- Post Graduate Training (Pharmacy Residency or Fellowship) or 3+ years of equivalent clinical pharmacy experience
How to Stand Out (Preferred Qualifications):
- Board certification by the Board of Pharmacy Specialties (BPS)
- Proficiency with Microsoft Office Tools and other technical applications (i.e. EMRs)
