What to Expect (Job Responsibilities):
- Provide care coordination services to members, including disease management and case management
- Educate and empower members to manage their health conditions effectively
- Assist with triage, referral, and transition of care planning for members with acute and complex healthcare needs
- Collaborate with physicians to ensure comprehensive patient care
- Manage member health plan benefits and assist in finding alternative services when necessary
What is Required (Qualifications):
- 5 years of clinical nursing experience
- Current unencumbered Registered Nurse License in the state of residency upon hire
- Experience working collaboratively with physicians in patient care management
How to Stand Out (Preferred Qualifications):
- Bachelor's Degree in Nursing or health education
- Current nursing experience in areas such as cardiology, endocrinology, pediatrics, obstetrics, oncology, or respiratory health
