What to Expect (Job Responsibilities):
- Conduct pre-visit reviews of selected ambulatory encounters to identify opportunities for recapturing medical conditions meeting HCC criteria
- Accurately interpret clinical information in medical records to identify potential diagnoses
- Educate providers on HCC considerations to enhance diagnostic specificity
- Identify education opportunities for providers based on chart review themes and feedback
- Collaborate with physicians and care teams to improve medical record documentation through effective communication
What is Required (Qualifications):
- Minimum three (3) to five (5) years of experience in case management, outpatient coding, utilization review, or CDI
- Strong understanding of clinical documentation and coding practices
- Current certification in Clinical Documentation Improvement (CDIP, CCDS, CCDS-O, or CDEO) preferred
- Certification in medical coding or risk adjustment (e.g., CRC, CPC, CCS, CDEO)
- Strong PC skills, including proficiency in Microsoft Office applications
How to Stand Out (Preferred Qualifications):
- Two years of experience in Primary Care, medical coding, risk adjustment, or CDI
- Medical licensure (RN, PA, NP) preferred
- Bachelor’s degree in a healthcare-related field preferred
