Why work as a Community Health Worker with Main Line Health?· Make an Impact! The Community Health Worker will be a key member of the health care team working to provide comprehensive, coordinated CHW interventions for Main Line Health patients. As a trusted member of their community, the CHW will work on the ground to promote healthy behaviors, build stronger relationships between the community and health and social services, and increase accessibility to those services.
Position: Community Health Worker- Part time 32 hours/week
Shift: Monday- Thursday- 8am-4:30pm- LMC home site- will travel to community partners based in West Philadelphia.Experience:
Minimum 2-years' work or volunteer experience in human services or healthcare.
Education:
High School diploma or equivalent requiredLicensure/Cert: Pennsylvania State Certification for Certified Community Health Workers or obtained within two years of hire
