      Job summary

      Community Nurse Learning Disabilities

      Band 6 AFC

      Permanent contract

      An exciting opportunity has arisen for an experienced RNLD to work as part of The Community Learning Disabilities team in Nuneaton covering North Warwickshire.

      The role involves close collaborative working with multi-disciplinary colleagues in the community setting, Managing own caseload, Supervising others, completing assessments, raising awareness of the health needs of adults who have a learning disability, identifying unmet health needs and providing specialist nursing advice.

      The successful candidate will need to be proactive, self-motivated and have experience of autonomous working, whilst also working under the direction of the community team manager.

      Main duties of the job

      The successful candidate will also be able to use current best practice/guidance/legislation to gain the best outcomes for clients and their carers. Will work within the MDT also assist in improving links with primary care, provide support, information and education to clients, carers and provider's and also deliver education to people with learning disabilities around health needs both on 1:1 or group basis.
