      What to Expect (Job Responsibilities):
      - Conduct assessments to evaluate the needs, strengths, and safety of individuals with disabilities and their families
      - Collaborate with community service departments and agencies to identify and provide necessary supports and services
      - Develop and implement person-centered community support and service plans
      - Complete required documentation for assessments and monitor progress towards goals
      - Respond effectively to crisis situations and coordinate crisis management plans

      What is Required (Qualifications):
      - Bachelor's degree in Social Work, Psychology, Sociology, Human Services, Special Education, or a related field
      - One year of full-time professional experience or two years of full-time para-professional experience in a human service agency
      - Valid Driver's License
      - Knowledge of human development, mental health, and community-based services
      - Ability to communicate effectively both orally and in writing

      How to Stand Out (Preferred Qualifications):
      - Master's degree in Social Work, Psychology, Sociology, Special Education, or a related field
      - Experience in home or community-based settings
      - Familiarity with MnCHOICES assessments and working with individuals eligible for related waivers
      - Experience engaging with diverse and underserved communities
      - Professional licensure related to working with individuals with disabilities
