      What to Expect (Job Responsibilities):
      - Conduct needs assessments and evaluate the safety and risk levels of individuals and families
      - Engage with persons with disabilities through virtual or in-person case management
      - Identify and coordinate Home and Community Based Services (HCBS) for individuals served
      - Develop and implement individualized community support and service plans
      - Monitor progress towards goals and adjust service plans as needed

      What is Required (Qualifications):
      - Bachelor's degree in Social Work, Psychology, Sociology, Human Services, Special Education, or a related field
      - One year of full-time professional experience or two years of full-time para-professional experience in a human service agency
      - Current licensure as a Registered Nurse in Minnesota (if applicable) and registration as a Public Health Nurse in Minnesota
      - Valid Driver's license
      - QDDP status for work with Rule 185 Developmental Disabilities (if applicable)

      How to Stand Out (Preferred Qualifications):
      - Master's degree in a relevant field
      - Experience in Public Health Nursing or supporting individuals with emotional, behavioral, and developmental disabilities
      - Experience working with children and adults with high medical or physical health conditions
      - Knowledge of community resources and funding sources related to disability services
