      What to Expect (Job Responsibilities):
      - Assess the needs, strengths, and family dynamics of individuals/families to evaluate safety and risk levels
      - Engage with persons with disabilities through virtual or in-person case management
      - Identify and coordinate Home and Community Based Services (HCBS) and additional supports
      - Develop and implement individual community support and service plans using person-centered planning
      - Respond effectively to crisis situations and develop safety plans for individuals served

      What is Required (Qualifications):
      - Bachelor's degree in Social Work, Psychology, Sociology, Human Services, Special Education, or a related field
      - One year of full-time professional experience or two years of full-time para-professional experience in a human service agency
      - Current licensure as a Registered Nurse in Minnesota and registration as a Public Health Nurse in Minnesota (if applicable)
      - Valid Driver's License
      - QDDP (Qualified Developmental Disabilities Professional) status if working with Rule 185 Developmental Disability

      How to Stand Out (Preferred Qualifications):
      - Master's degree in Social Work, Nursing, Psychology, Sociology, Special Education, or related field
      - Experience supporting children and adults with emotional, behavioral, and developmental disabilities
      - Experience in supporting individuals with high medical and/or physical health conditions
      - Familiarity with cultural diversity and equity initiatives
      - Experience engaging with underrepresented communities
