      What to Expect (Job Responsibilities):
      - Facilitate and coordinate care for assigned members through assessments and collaboration
      - Develop and implement individualized care plans in partnership with members and their care teams
      - Provide health education and promote preventative care to support member wellness
      - Coordinate delivery of care across various healthcare settings and community resources
      - Evaluate outcomes of care coordination and ensure compliance with regulatory requirements

      What is Required (Qualifications):
      - Master’s Degree in Social Work required
      - Minimum of three years of direct member care experience in acute, ambulatory, or community-based care
      - Ability to establish and maintain member-centered relationships
      - Knowledge of emotional and social factors that impact patient health
      - Must be available to work during day shifts

      How to Stand Out (Preferred Qualifications):
      - Bilingual in English/Spanish is preferred but not required
