Hold a caseload of clients, providing direct support to help them live well with dementia.

Carry out visits in people's own homes and care home settings across North Devon.

Offer tailored support to those with more complex presentations, including working closely with carers and families.

Use the DiADeM (Diagnosis of Advanced Dementia) model to carry out assessments, with guidance from our experienced multidisciplinary team.

Collaborate closely with professionals across the team and link in with local dementia services and voluntary sector partners.
- Experience of working in a health or social care setting.
- Working with people with challenging behaviour and complex health needs
- Knowledge of Dementia and person-centred care.
- An understanding of physical health problems which can impact a persons mental health and monitoring of the same.
- Knowledge of safeguarding principles and appropriate action to take.
- Awareness of the Code of conduct for Health Care Support Workers in England.
- GCSE in Maths and English (Grade C or above) OR Level 2 Key Skills / Functional Skills in Maths and English.
- QCF Level 4 or equivalent level of knowledge or substantial knowledge from experience in a caring role.
