      Job summary

      This is an exciting new role within the Mid Devon Older Peoples' Mental Health team. You will be part of a well established team with a focus on improving the lives of people with dementia following their diagnosis and throughout their journey.

      You will work with people living in the community in their own homes and in care home settings to support them to live well with dementia and offer carer support to those around them.

      The Older Peoples' Mental Health directorate is a friendly and welcoming team who will support you in all aspects of your new role.

      Main duties of the job

      You will work with a caseload of people and support them and their carers after receiving a diagnosis of dementia. This could mean helping them receive all the appropriate information and services available to them

      You will be expected to visit people in the community in where they are living.

      Support people within care homes who may have more complex presentations including non-compliance with care and treatment, working with them and carers to find personalised solutions to enhance care and treatment.

      You will also be involved in working with people who have lived with dementia for some time and require additional support and specialism to remain at home.

      You will work with other professionals within the team and link in with dementia services within the community and voluntary sector.
