      What to Expect (Job Responsibilities):
      - Conduct phone and video meetings with individuals, families, and caregivers to assess needs and eligibility for services
      - Perform structured interviews and document assessments accurately in report templates
      - Provide information regarding services, eligibility criteria, and wait periods
      - Serve as an ongoing resource for individuals seeking updates and information
      - Participate in team meetings and training activities to enhance skills and knowledge

      What is Required (Qualifications):
      - Experience supporting individuals with intellectual and developmental disabilities
      - Bachelor's degree in Social Work, Psychology, Special Education, or related healthcare/human services field
      - Strong computer proficiency and effective communication skills
      - Outstanding organizational skills and ability to work autonomously
      - Residency in Oklahoma required

      - Familiarity with home and community-based services for individuals with disabilities
      - Ability to communicate effectively with diverse populations
