      Job summary

      An exciting opportunity has arisen at Moore Street Surgery for an experienced Diabetic Nurse Practitioner to join our team for 6 months with a possible view to an extension following review of the pilot.

      Main duties of the job

      We are looking to recruit an experienced Diabetic Nurse Practitioner, Band 7 or above. Considered applicants require to have a valid nursing qualification and registration with a relevant regulatory body and a minimum of 3 years experience in specialised diabetes care (mandatory). The successful candidate needs to be able to work as as an autonomous practitioner and have advanced diabetes knowledge and excellent communication skills. In addition the role requires supporting upskilling of the practice staff to raise standards of general diabetes care and awareness and to support QOF and LQC requirements. This post is a pilot project to enhance diabetic care and will be reviewed after 6 months with a view to extension. The pilot has been undertaken with the primary aim of addressing all diabetic care but particularly targeting a sub-group of diabetic patients deemed high risk with the aim of optimising glycaemic control and proactively being involved in education of patients and the clinical team at Moore Street alike. This post, as previously stated, can be undertaken alongside a current NHS post. Renumeration is dependant on experience but pay scale is £35 - £40 per hour for 4-8 hours weekly.
