      Job summary

      Durham West Primary Care Network are recruiting a Digital Transformation Lead to join the PCN management team and support delivery of the local and national agenda on digital transformation. The successful candidate will be highly skilled and motivated with experience of working in a primary care setting.

      For the right candidate the PCN will consider full time, part time or job share working arrangements.

      Main duties of the job

      The Digital Transformation Lead (DTL) will manage, maintain, and support existing projects and infrastructure across the PCN, ensuring they continue to deliver measurable benefits.

      The DLT will be key in understanding the PCN/primary care digital requirements and delivering on their responsibilities in the Fit for the Future 10 Year Health Plan for England.

      The role will also involve scoping, planning, and delivering new initiatives, with a strong emphasis on enhancing digital capabilities and optimising digital and clinical systems.

      About us

      Durham West Primary Care Network is an NHS collaboration between 3 GP Practices: Chastleton Medical Group, Dunelm Medical Practice, and The Medical Group with a combined patient population of 58,220 population.We work together to provide extended services to patients registered at our 9 surgery sites.

      The successful candidate will join an experienced PCN management team and engaging practice staff who work together to deliver high quality, effective primary care medical services to the patient population they serve.

      Job description

      Job responsibilities

      The DTL will have the following responsibilities:-

      Transformation Projects

      Oversee and maintain the delivery of existing digital transformation projects ensuring sustainability and ongoing benefits.

      Scope, plan, and implement transformation initiatives, working with clinical and management teams to ensure alignment with PCN priorities.

      Monitor and evaluate the outcomes of transformation projects, producing reports and recommendations for improvement; present findings to stakeholders at PCN meetings.

      Draft and prepare reports to support financial claims and ensuring timely submission to the Integrated Care Board.

      Support the PCN and member practices to develop (where necessary), maintain and update relevant policies and procedures.

      Digital Support and Infrastructure

      Ensure implementation of any new systems, digital platforms or system add ons comply with NHS England, GDPR and cyber security requirements.

      Training and Digital Capability

      Provide induction, training and guidance to staff on the use of clinical systems, digital tools and IT processes.

      Data and Reporting

      Support practices in using business intelligence tools to extract, interpret and act on data.

      Promote high standards of data quality and integrity across the network.

      Collect data, intelligence and information to report to and advise the PCN Board on performance against all local, PCN DES and the 10 Year Plan measures.

      Collaboration and Communication

      Work in partnership with PCN leadership, practice managers, and other key stakeholders to ensure smooth project delivery and effective digital systems and operations.
