RESPONSIBILITIES:
Manage the development and implementation PHM Care Continuum Operations:
· Define/refine, implement, and lead the PHM care continuum strategy
· Lead and manage the scoping, design, and implementation of specific PHM interventions. Activities may include:
o Set and manage to priorities and Key Performance Indicators (e.g., LOS, readmission etc.)
o Develop operational processes, management structures, and governance models
o Build and manage budget, staff, and key stakeholders
· Facilitate development of project roadmaps and strategy for services with administrative and clinical leadership, management, and practice stakeholders
· Maintain knowledge on care continuum industry best practices including but not limited to post-acute landscape, PAC network management, and patient discharge management
Lead and direct Care Continuum services:
· Provide operational leadership of PHM care continuum programs including the MGB SNF Collaborative, SNF Transitional Care Management program, and 3-Day Waiver
· Drive the execution of best practices in operational processes, policies, data and technology
· Direct and operate the functional components of the interventions across the clinical spectrum including the following elements:
o Oversee evaluation and performance measurement
o Revising and enforcing policies, processes, and communication plan as need
o Managing of the hiring and overall talent management
o Manage iterative process improvements to optimize outcome and satisfaction
o Track and communicate teams’ progress to stakeholders, collaborators, and leadership
· Anticipate, identify, and drive resolution of intervention challenges
· Assess and manage risk during scope of project throughout lifecycle-adjusting outcomes
· Identify and recommend policies and procedures as needed to ensure quality, compliance, accuracy, and cost efficiency
Use data to drive decisions and manage performance
· Develop and expand existing excel models for assessing the financial impact of proposed and on-going PHM services (e.g., develop business cases, monitor revenue & expenses)
· Track performance and perform trend analysis to monitor and meet PHM key performance indicator (KPI) targets
· Analyze data and processes to enable performance improvement initiatives (e.g., discharge rates, LOS, network quality, readmission rates etc.)
· Implement evidence-based interventions, ensuring constituents are represented, targets and goals are identified and documented, and effective communication
· Manage budgets to ensure that expenses are in line with expectations
Provide high performance leadership
· Provide leadership and management oversight of interventions and staff
Foster collaborative relationships across the enterprise
· Build trusting relationships across PHM, MGB, as well as externally to ensure collaborative and robust strategic thinking
· Partner with and educate system leadership and physicians on care continuum strategy, priorities and implementation models
· Ensure alignment and collaboration with key external stakeholders (e.g., SNF and home health facilities)
· Approach conflict in a constructive manner, helps identify problems, offer solutions, and participate in resolution
· Participate and accept ownership for cross-department initiatives

!*!Qualifications:
· Bachelor’s degree required
· Advance degree in Case Management, Medicine, Business Administration, Health Care Administration and/or comparable of study preferred
· At least 10 years’ experience in health care, preferably in a hospital or post-acute care setting
· At least 5 years in a leadership role
· Knowledge and experience in post-acute care, care continuum and/or hospital health care operations required

Working Conditions:
· A hybrid work model, with 2 days per month on-site; professional office setting located in Assembly Row, Somerville, MA.
