      This role is a key part of the quality leadership team for the system and the senior MGH administrative leader for quality performance and improvement within Mass General Brigham home care. This role will report directly to the MGB Associate Chief Quality Officer and/or Chief Quality Officer and is responsible for the success of a broad and sweeping quality program within Mass General Brigham Home care. This role will partner with the system quality team as well as the broader Health Care at Home team to execute a robust and comprehensive quality strategy within MGB HealthCare at home. This role will oversee all clinical compliance, quality, safety, performance improvement, infection control and risk management activities within home care. This role will lead a team of local quality team members who focus on day-to-day oversight, coordination, execution, and implementation of all quality activities at the site. This role will lead day-to-day oversight, coordination, execution, and implementation of all quality activities associated with the MGB Home Care including primary responsibly for quality components of the CMS Patient-Driven Groupings Model (PDMG), CMS Home Care Quality and Experience Star Ratings, and CMS Home Care Value-Based Purchasing. This role will also ensure compliance with federal, state, and local regulatory requirements and accreditation standards.

      Responsibilities:

      · Provide essential input to the MGB leadership team to create the strategy, policies, infrastructure, QAPI plans, and performance improvement plans to ensure that MGB Home Care delivers the highest quality of care in the nation.

      · Execute the MGB quality strategy at MGH, ensuring adherence to MGB standardized processes around quality measurement, strategy, prioritized areas, and performance improvement

      · . Build out and stabilize the personnel and infrastructure for the quality team at MGB home care, ensuring that all team-members follow standardized and best-practice procedures and approaches within all domains. Perform local management and review of these staff.

      · Serve as the senior administrative leader within MGB Home Care responsible for performance on all CMS conditions of participation, government and private payer pay for performance activities in the quality and equity domains. Responsible for performance on key MGB priorities including improvement in mortality and reduction in serious reportable events. Identify areas at risk and with bidirectional communication and planning with MGB leadership, develop performance improvement planning to ensure we achieve all quality goals within home care.

      · Responsible for partnering with the MGB data and analytics team to ensure that reporting to CMS and other regulatory bodies, private payer contracting, external benchmarking bodies, etc. is timely and correct.

      · Advise in the creation of any needed MGB Home care dashboards around quality performance and improvement.

      · Monitor national advancements and changes in regulatory standards and advise on any evolutions to reporting or benchmarking within the Home Care space.

      Specific activities and coordination required:

      o Liase as needed with the MGB VP of Patient Safety, leading MGB Home Care Safety teams to create prospective solutions and performance improvement as a result of safety events. Lead prospectively to create performance improvement in pursuit of harm avoidance.

      o Liase as needed with the MGB Director of Infection Control and local team to monitor and improve performance and respond to data to lower the risk of infections to our patients.

      o Liase as needed with the local clinical documentation specialist team to ensure clinicians are capturing the patient’s burden of disease and disease complexity.

      o Ensure that all regulatory standards and regulations are met or exceeded. Serve as clinical compliance officer for home care.

      · Partner with the Healthcare at Home Chief Operating Officer & Chief Nursing Officer and the Chief Clinical and Innovation Officer as needed to execute system goals and site level improvements.

      · Provides proactive and reactive quality performance improvement plans, and partner with local teams and department or floor-based teams to improve or implement key improvements as needed. Able to effectively and persuasively communicate goals, strategic planning, and accountability processes with department chairs and enlist the partnership of departmental teams for success.

      · Prepares reports and presentations as needed; Oversee local training and educational programs as needed.

      · Understand data provided by the system team and use knowledge of local practices and culture to identify areas of opportunity for improvement as well as communicate to the system areas which are at risk.

      · Provides strong bi-directional communication between the system and the site, with accountability for distribution of organization communication (including pertinent data and analytics) from MGB-to-MGB home care and vice versa.

      · Must be available to work in the case of a hospital declared emergency. Must be available to assist during regulatory agency reviews.

      Qualifications:

      · Master’s Degree, Field of Study: Nursing, Public Health, Business Administration, Public Administration or Health Services Administration.

      · Minimum of 5-7 years leadership in Quality and managing large teams at a Certified Home Care Agency.

      · Experience and comfort in managing direct reports and ability to grow those staff professionally.

      · Demonstrated knowledge and application of principles of quality measurement, governmental and regulatory quality requirements, process improvement for a certified Home Care agency. Demonstrated knowledge and competence in executing and sustaining improvements in clinical care, as well as holding themselves and other stakeholders accountable to sustaining improvement over time. Ability to proficiently explain these domains to others, who may or may not have expertise within rigorous quality improvement.

      · Demonstrated ability to successfully lead a local quality performance and improvement team, as well as demonstrated competence and knowledge in clinical home care which allows them to partner with collaborators to achieve key objectives.

      · Strong background and skill in implementation science, and the ability to move from measuring a problem to correction of the problem with an eye towards sustainability and accountability.

      · Ability to balance the need for collaboration vs the need to make executive decisions quickly and reliably.

      · Knowledge of requirements for maintaining regulatory accreditation and standards compliance throughout diverse and specialized areas that are relevant to home care.

      · The ability to be on site and flexibility with different locations.
