      Main duties of the job

      We are looking for a discharge and transfer coordinator to join the cardiovascular and thoracic care group. You will be part of the bed management team and will focus on ensuring safe and timely discharges for our patients. You will be expected to work Monday to Friday but should be willing to flex according to acuity within the ward areas.

      The cardiovascular and thoracic care group is the regional centre for cardiac surgery, cardiology, vascular and thoracic surgery. We provide a wide range of specialist adult and paediatric services. The care group is a welcoming, busy and dynamic environment and this is an exciting opportunity for an enthusiastic, motivated, self-directed individual to become part of the cardiovascular and thoracic bed management team.

      You will be responsible for Co-ordinating all discharge plans of patients within the cardiovascular and thoracic footprint working with the multi-disciplinary team and assisting in managing effectively and efficiently the use of beds within the Care Group. As part of the service, you will be well supported by the ward teams and management alike, with regular team meetings and discussions which may lead to improvements in the service.

      WHAT WERE LOOKING FOR

      We are looking for someone who is articulate, proactive and motivated and has excellent interpersonal skills.

      Combined with NHS experience, you will need to have the ability to multi-task and organise your own workload whilst remaining calm under pressure and enjoy a challenge.

      This is a varied role, requiring an organised approach, attention to detail and strong skills in communication, and administration. Please specify in the supporting statement the preferred hours/days you would wish to work.
