- You will work closely with teams focusing directly on the needs of patients requiring support and facilitation in order to be discharged from hospital.
- You will develop close working relationships with ward teams providing coordinated support to the discharge hub.
- Responsible for assisting with processes and procedures to enhance patient flow.
- Providing follow up as appropriate for patients discharged taking action and/or escalating issues as required.
- Travel easily between coast and countryside, with a blend of picturesque villages and seaside towns there's always new experiences to be enjoyed
- Embrace the city life with great access to visit Brighton and London
- With easy access to Gatwick and Heathrow and excellent railway links across the UK there is plenty of opportunity for adventure
- Satisfactory level of general education

Working as part of the discharge hub covering Meadowfield Hospital and the Forget-Me-Knot Unit in Worthing the Discharge Support Worker will ensure that all discharges from a defined number of mental health acute, dementia, rehab and older person's beds are processed and coordinated in such a manner to ensure that patients are placed in an environment best suited to their assessed needs and Trust resources are utilized to their maximum efficiency. The post holder holds no clinical responsibility instead focuses on the administrative coordination to support successful discharge.
We are looking for an individual who is resilient, driven and passionate to provide high quality care to our patients.

You will work closely with ward staff, Trust teams and liaise with relevant agencies to coordinate the administrative procedures associated with discharge.
You will liaise with patients and carers and support risk identification.
