What to Expect (Job Responsibilities):
- Conduct and complete face-to-face comprehensive assessments and reassessments at clients' residences
- Develop initial and ongoing care plans to support clients in community settings
- Provide case management services in accordance with established policies and manuals
- Make referrals to community resources and consult with supervisors on complex cases
- Act as a mandatory reporter for vulnerable children and adults, ensuring timely referrals to appropriate services
What is Required (Qualifications):
- One year of experience as a Social Service Specialist 2 or equivalent
- Master’s degree in social services, human services, behavioral sciences, criminal law/justice, or an allied field with one year of relevant experience
- Bachelor’s degree in the aforementioned fields with two years of relevant experience
- Six years of paid social service experience performing functions equivalent to a Social Service Specialist 2
- Ability to complete assessments in clients' homes and perform physical requirements of the job
How to Stand Out (Preferred Qualifications):
- Knowledge of Medicaid terminology, regulations, and policies
- Experience in case management and community resource referrals
- Proficiency in word processing and data input skills
- Ability to analyze programmatic issues and develop effective solutions
- Familiarity with software applications such as Word and Outlook
