What to Expect (Job Responsibilities):
- Serve as the first line of customer service for external and internal customers
- Professionally respond to electronic referral platforms within specified time requirements
- Receive and document patient referrals to home health services through electronic systems
- Coordinate with care centers to ensure staffing for electronic referrals
- Communicate pertinent patient information to the care center team and follow up on pending admissions
What is Required (Qualifications):
- High School diploma or GED equivalent
- Experience in an environment where timeliness and quality of response were measured
- Experience in home health or hospice services
- Customer-facing sales experience is preferred
How to Stand Out (Preferred Qualifications):
- Prior experience in home health or hospice care
- Background in customer service or sales roles
