  What to Expect (Job Responsibilities):
  - Facilitate completion of care evaluations in nephrology offices, dialysis clinics, or via telehealth
  - Perform outbound calls for scheduling, reminders, and rescheduling of care evaluations
  - Welcome new patients and collect information on their concerns to escalate as needed
  - Maintain accurate documentation and communicate updates to the care team
  - Coordinate care by ensuring access to eligible resources and assisting with referrals
  
  What is Required (Qualifications):
  - High School Diploma or GED required; Associate’s or Bachelor’s Degree preferred
  - Minimum of 2 years of professional experience in customer service, patient-facing, or telephonic outreach roles
  - Prior healthcare experience preferred
  - Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook
  - Exceptional verbal and written communication skills, along with strong organizational abilities
  
  How to Stand Out (Preferred Qualifications):
  - Experience in a healthcare setting or with telehealth services
  - Familiarity with motivational engagement techniques
  - Strong time management skills and ability to work autonomously
  - Experience in coordinating care or working with chronic conditions
