      What to Expect (Job Responsibilities):
      - Facilitate completion of comprehensive health evaluations (CHE) in nephrology offices, clinics, or via telehealth
      - Perform outbound calls to patients and caregivers for scheduling, reminders, and rescheduling of CHEs
      - Welcome new patients and collect concerns for escalation to nursing staff as needed
      - Maintain accurate documentation and communicate updates to the care team
      - Organize outreach efforts by urgency and importance for maximum patient impact

      What is Required (Qualifications):
      - High School Diploma or GED required; Associate’s or Bachelor’s Degree preferred
      - Minimum of 2 years of professional work experience in customer service or telephonic outreach
      - Prior experience in a healthcare setting preferred
      - Intermediate proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook)
      - Strong verbal and written communication skills, along with excellent organizational abilities

      - Proven ability to work autonomously and collaboratively with team members
