      What to Expect (Job Responsibilities):
      - Develop, oversee, and manage patient care according to the physician-approved Plan of Treatment
      - Conduct telephonic or face-to-face clinical assessments to identify and manage patient needs
      - Communicate physician's interim orders and ensure EMR approval and oversight
      - Work collaboratively with patients, providers, and healthcare professionals to support treatment adherence
      - Ensure compliance with regulatory, accrediting, and company policies and procedures

      What is Required (Qualifications):
      - Active RN license in Virginia and North Carolina
      - Minimum of 2 years of nursing experience, including 2 years in home health, hospice, or home infusion
      - Associate's Degree in nursing
      - Basic Life Support (BLS) certification required within 90 days of hire
      - Experience within the past 5 years in home health services

      How to Stand Out (Preferred Qualifications):
      - Experience with telehealth or remote patient monitoring
      - Familiarity with electronic medical records (EMR) systems
      - Strong communication and interpersonal skills
      - Ability to work independently and manage time effectively
      - Previous experience in a patient advocacy role
