What to Expect (Job Responsibilities):
- Provide virtual urgent and longitudinal chronic care management for new and established patients
- Collaborate within a care team approach in Family Medicine
- Utilize digital diagnostics and ancillary support to enhance patient care
- Engage in video visits for patients across more than 20 primary care practices
- Maintain a strong virtual presence and contribute innovative solutions to healthcare challenges

What is Required (Qualifications):
- Board Certified Family Medicine or Adult/Geriatric Advanced Practitioner
- Minimum of 3 years of experience in Primary Care; Urgent Care experience also accepted
- Strong solution-oriented mindset and ability to think outside the box
- Passion for innovation in healthcare delivery
- Must be willing to support multi-state licensing requirements
