What to Expect (Job Responsibilities):
- Conduct in-home assessments 3 days a week and work from home 2 days a week
- Obtain comprehensive history, physical exam, medication review, and appropriate cognitive/depression/safety screenings during assessments
- Engage in end-of-life care discussions with patients and families
- Collect laboratory specimens in the home setting when appropriate
- Communicate findings of patient assessments to inform the primary care provider of potential gaps in care
What is Required (Qualifications):
- Active California state Nurse Practitioner or Physician Assistant license
- Active Nurse Practitioner Board Certification/Physician Assistant Certification
- NPI Number, DEA, and valid BLS certification
- Valid California driver license and current automobile insurance
- Master's degree from an accredited NP Program or PA program
How to Stand Out (Preferred Qualifications):
- One (1) year of prior clinical or home care experience
- Previous EMR experience
- Experience in the care of older adult (geriatric) patients
