      What to Expect (Job Responsibilities):
      - Establish and maintain effective relationships with patients, caregivers, and healthcare providers
      - Perform comprehensive assessments on newly enrolled patients and provide ongoing care
      - Develop and update patient management care plans based on condition changes
      - Conduct post-discharge visits to ensure patient stability and alignment with care goals
      - Participate in clinical after-hours on-call program on a rotational basis

      What is Required (Qualifications):
      - Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
      - Current, unrestricted NP or PA licensure in the State of Practice
      - Active, unrestricted RN licensure in the State of Practice for Nurse Practitioners
      - Access to reliable transportation and compliance with the employer's Motor Vehicle Safety policy

      How to Stand Out (Preferred Qualifications):
      - 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or similar settings
      - Knowledge of long-term care, community resources, and patient care delivery systems
      - Familiarity with CPT, ICD-10, and HCPC codes
      - Awareness of NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts
