Job summary
We are looking for someone who can support people with frailty and the ongoing implementation of integrated neighbourhood teams and MDT working across both boroughs. The postholder will lead the evaluation of the project particularly in relation to impactthrough the collection of data, both qualitative and quantitative.
Work in collaboration with Health Care Professionals and to identify those at risk of deterioration, deconditioning or admission for discussion with the MDT, initiate or review of advance care plans through the UCP.
You will work in a small team, but also link in with the other community teams based at Your Healthcare.
Main duties of the job
This is a fixed term post and is for 37.5 hours per week. The usual working hours are 9-5 and will be Monday Friday
You will be mostly based at Hollyfield House, Surbiton although you will also need to be able to travel to our other sites as needed.
