- Demonstrate an ability to deliver effective telephonic coaching in a manner that promotes behavior change for the NJ Developmentally Disabled population
- Be able to assist clients in creating a healthy vision and goals for their Health & Wellness program
- Participate in weekly meetings and monthly in-service training
- Appropriately adapt telephonic coaching and education methods to accommodate the unique physical, psychosocial, cultural, age-specific needs of each clientele served
- Demonstrate established customer service behaviors and standards
- LPN, MSW, or BSW with active and unrestricted NJ license / certification
- 3+ years of clinical experience
- 2+ years of experience in direct education and / or coaching with individuals for disease prevention, health promotion and behavior change
- 1+ year experience with electronic documentation
- Proficient in PC-based word processing and database documentation (Word, Excel, Internet, Outlook) and the ability to type and talk at the same time
- Basic knowledge of disease prevention and basic understanding of exercise principles, nutrition and weight management
- Access to reliable transportation and the ability to travel up to 25% within assigned territory to meet with employees, members, and providers
- Experience serving the special needs population
- Managed Care experience
- Experience with government health programs (Medicaid / Medicare)
- Ability to navigate multiple computer programs simultaneously
- Ability to speak Spanish

This metrics-driven Health Coach role requires telephonic outreach to UHC members that have been pre-identified. Members enrolled in this program are primarily developmentally disabled, medically complex and require intensive medical and psychosocial support. Coordination of enrollee care needs across the continuum begins with early identification of health risk factors, education, and prevention using the six essential activities of case management as per CMSA. Coordination also assures the provision of appropriate services in acute, home, chronic and alternative care settings, to meet the needs of members in the most cost-effective manner available.
This is a work at home position with up to 25% travel within New Jersey. If you are located in the state of NJ/PA/NY, you will have the flexibility to telecommute* as you take on some tough challenges.

New Jersey or New York Residents Only: The hourly range for New Jersey residents is $23.22 to $45.43 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
