The Health Education Specialist (Care Coordinator) for the Psychiatric Research Institute (PRI) Child Diagnostic Inpatient Unit (CDU) uses critical skills to gather patient information from multiple sources for new child inpatient admission.
Summary of Job Duties:
The Health Education Specialist helps to coordinate and submit clinical information for Prior Authorizations to the PASSEs for admission to the Child Diagnostic Unit. The Health Education Specialist will have to understand processes of CDU admission, assessment, treatment and discharge, yet also have an understanding of Arkansas Medicaid, the PASSEs, and TEFRA. The Health Education Specialist coordinates admissions, care and discharge by speaking with the patient’s family members, providers, and schools. If needed, they will help coordinate discharge care including sending referrals for hospitalization recommendations (residential, Optum/PASSE screening, new outpatient providers). They will meet with families at admission and obtain release of information for all outpatient providers, schools, and past hospitalizations.
Duties may include, but are not limited to: office administration and organization, arranging outpatient services, making outpatient follow-up appointments, sending confidential patient records. Participate in treatment team for information sharing and discharge planning. Patient contact may include reviewing insurance documentation for signature, confirming patient family contact information or collecting other patient information necessary for discharge planning. The Education Specialist will send discharge reports to outpatient providers and schools. The Education Specialist will assist families with reintegration into the community by contacting schools or outpatient treatment providers with advice for implementing treatment recommendations when needed. The Education Specialist will have access to patient electronic medical records and will participate in quality improvement and staff development activities at the department level. The Education Specialist receives direction from the CDU Medical Director.
