Job summary
An exciting opportunity has arisen within Shropshire Community Health Trust to appoint an enthusiastic Health Visitor to join the Healthy Child Programme 0- 19 Telford Service. The post will be for the Community First Team, this team is dedicated to delivering the universal and universal plus elements of the Healthy Child Programme and early preventative interventions. The candidate will need to be flexible as they may be required to work within the family first team, who support those children and families who require a safeguarding response.

Main duties of the job
Our forward thinking innovative and dynamic Healthy Child programme 0-19 Telford service supports health and wellbeing programmes for all children and young people living in Telford and Wrekin. We are looking for a motivated individual with experience of working with children, young people and families. You should have the ability to lead and work within a skill mixed team and deliver allocated components of the Healthy Child Programme. The post holders will have a lead role within maximising the health of children, families and the community.
Your role will require you to work in partnership with partner agencies and providers, to promote and enhance the health and well-being of children and families. You must be able to meet the travel requirements of the post.
