      Thank you for considering this exciting opportunity. We are looking for enthusiastic and dynamic Health Care Assistant to join our friendly team working on our 24 bedded Complex Frailty Unit at Bishop Auckland Hospital.

      Main duties of the job

      To assist the qualified nursing staff, under their supervision, in providing a high quality, comprehensive and compassionate nursing service to all patients and users, ensuring a high standard of patient care is maintained in a clean and safe environment which promotes dignity, privacy and respect. You should demonstrate excellent caring and people skills. You must also demonstrate a good verbal and written command of English, along with excellent communication and interpersonal skills.

      Under appropriate supervision (direct or indirect), the post-holder will assist in the delivery of care planned and assessed by the Registered Nurse, including providing clinical care and performing a range of diagnostic tests and other extended skills.

      We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

      Job description

      Job responsibilities

      To contribute to the care planning process involving and informing patients and where appropriate their carers.

      Organise, prioritise and adjust own workload in relation to patient care

      Liaise with members of the multi-disciplinary team in the delivery of care programs

      To recognise and act upon deterioration in a patients condition informing a Registered Nurse of action taken

      To assess patients level of dependence in meeting personal care needs and ensure appropriate assistance is given

      To discuss with patients strategies to maximize independence

      To keep accurate and timely written records of care delivered and patient progression

      To assist medical staff and registered nurses in carrying out invasive procedures

      To take responsibility for ongoing education and their personal development plan

      To welcome patients to the unit, and facilitate patient transport needs to and from hospital

      To assist in the admitting of patients to the ward, including recording these on clinical systems, recording base-line observations.

      To provide care to patients following instructions within the Care Plan or Pathway

      To carry out clinical skills relevant to the unit which may include phlebotomy, recording ECGs, recording blood glucose, performing MRSA screening and monitoring/removal of IV cannulas
