What to Expect (Job Responsibilities):
- Provide crisis intervention services in a home-based environment with flexible scheduling
- Conduct comprehensive assessments of family strengths, needs, and barriers to goal completion
- Advocate for families and provide concrete services to meet their needs
- Collaborate with community resources and systems to support families
- Participate in program reporting and quality improvement processes
What is Required (Qualifications):
- Bachelor's degree or Master's degree with 1 year of relevant experience in community-based health or case management
- Preferred licensure: LMSW, LCSW, LMHC, LMFT, or LCAT
- Must possess a valid NYS driver's license
- Experience in the Homebuilder model of crisis intervention is preferred
