      Sentara Home Health in Dumfries, Suffolk, Gloucester, Newport News, Chesapeake, Virginia Beach, Charlottesville, Harrisonburg, and Halifax VA are seeking Registered Nurses and LPN's to join their team.

      Registered Nurse position:

      Overview

      The Direct Care Registered Nurse - Home Health and Hospice develops, implements, and evaluates plans of care for specific patient populations. Manages a patient case load through coordination and collaboration with multidisciplinary team members to facilitate integrated and comprehensive care. Possesses clinical knowledge and skills to meet standards as required by specific clinical areas. Basic Life Support (BLS) within 90 days of hire. A valid RN license in state(s) of practice is required.

      • Associate's Degree (Required)

      Certification/Licensure

      • RN (Required) - Active in Virginia
      • BLS (Required within 90 days)

      Experience

      • 1 year of nursing experience (Required)
      • Previous Home Care experience (Preferred)

      LPN Position:

      Overview

      The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being.

      • Trade School Graduate (Required)

      Certification/Licensure

      • LPN License (Required) - Active in Virginia
      • BLS (Required within 90 days)

      Sentara Home Health is looking for Physical Therapists in Charlottesville, Halifax, Newport News, Gloucester, and Dumfries, VA

      Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, and improve or correct disabling conditions resulting from disease or injury. Provides age-appropriate education for clients and caregivers. Supervise and assume responsibility for care provided by identified team members.

      Education

      • Bachelors Degree in Physical Therapy (Required)
      • Master's Degree
      • Doctorate Degree in Physical Therapy

      Certification/Licensure

      • Physical Therapy License (Required)

      Experience

      • 2 years of experience

      Home Health in Halifax/South Boston, VA is looking for an Occupational Therapist

      Overview

      Our highly skilled team of clinicians with Sentara Home Health Services partners with individual physicians to coordinate the multiple therapies and services each patient may require – from skilled nursing care and infusion therapy to respiratory care and medical equipment. Sentara Home Health Services is accredited by the Accreditation Commission for Health Care and belongs to a number of local, state and national organizations.

      Education

      • Bachelors
      • Masters in Occupational Therapy (Preferred)
      • Bachelors Degree + 2 years relevant experience may be accepted in lieu of degree

      Certification/Licensure

      • Occupational Therapist (Required)

      Experience

      • 2 years of experience as an OT required
