What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations
- Utilize technology for clinical workflows and documentation during evaluations
- Provide diagnostic and preventive services as indicated, including spirometry and bone density screenings
- Travel locally within licensed states to meet patients in their homes
- Build trusted relationships with patients to help bridge gaps in care
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover designated primary locations, such as Colorado Springs
- Comfortable performing duties in diverse home environments
- Flexibility to conduct virtual visits as needed
How to Stand Out (Preferred Qualifications):
- Multi-state licenses
- Experience conducting in-home health evaluations
- Familiarity with diagnostic and preventive services
- Strong communication skills and a passion for patient care
- Previous experience in a similar healthcare role
